The first and only FDA-authorized treatment for negative symptoms of schizophrenia.

Indication: Motivista is a prescription digital therapeutic indicated for the treatment of negative symptoms of schizophrenia as an adjunct to clinician-managed outpatient care, including antipsychotic therapy, in patients 18 years of age and older with clinically stable positive symptoms.

Clinical evidence

Backed by a rigorously designed pivotal study.

Motivista (CT-155) received FDA Breakthrough Device Designation and was then authorized on the basis of CONVOKE, a rigorously designed pivotal study. A pragmatic study, ENSPIRUS, is ongoing.

The CAINS (Clinical Assessment Interview for Negative Symptoms) is a semi-structured clinical interview used to assess negative symptoms of schizophrenia. Its motivation and pleasure subscale (CAINS-MAP) was the primary endpoint in CONVOKE.

CAINS-MAP is a 9-item clinician-rated subscale of the CAINS (range 0–36); a reduction indicates improvement in negative symptoms. The primary effectiveness endpoint (change from baseline to Week 16 in CAINS-MAP total score) was analyzed using a prespecified Mixed Model Repeated Measures (MMRM) approach on the intent-to-treat (ITT) set with observed data, i.e., that included subjects with at least 1 post-baseline CAINS-MAP assessment (Week 8 and Week 16). The number of participants whose longitudinal data directly inform the primary MMRM is 196 (Motivista) and 204 (Digital control). Two prespecified sensitivity analyses were conducted using the full ITT set with imputation (N=457; 227 Motivista + 230 Digital control). Both sensitivity analyses, conducted on the complete N=457 ITT population, corroborated the primary result, supporting that the treatment effect estimated from the 196/204 MMRM analysis population is representative of the full ITT population.

The Motivista clinical development program also includes an ongoing prospective pragmatic study, ENSPIRUS (NCT06791122), evaluating Motivista's effectiveness, engagement, and safety in routine clinical practice. Early phase research includes published exploratory studies establishing early feasibility (an exploratory 7-week feasibility study) and formation of the Digital Working Alliance (JMIR Mental Health, 2025).
